Dr. Kourtney Osorio is a board-certified dedicated, and accomplished Family Medicine physician. She completed her undergraduate education at the University of California, Davis, graduating with a Bachelor of Science in Biotechnology. Following her undergraduate studies, Dr. Osorio pursued her passion for medicine, earning her Doctor of Medicine (M.D.) degree from the University of California, Irvine School of Medicine. Her extensive training and licensure show her commitment to medical education and patient care. Dr. Osorio completed her Residency in Family Medicine at Long Beach Memorial Hospital and holds a board certification from the American Board of Family Medicine. She currently has held academic appointments as an Associate Professor of Medicine at both Georgetown University and Temple University. In her professional roles, Dr. Osorio has served as an Attending Physician at Providence Hospital in Washington, D.C., and Temple University Hospital in Philadelphia, PA. Her dedication to medical education extends to her role as a Teaching Attending in Family Medicine at various institutions. Dr. Kourtney Osorio, M.D., is a prominent figure in the field of Family Medicine, known for her unwavering commitment to patient care, medical education, and her remarkable achievements in the medical community.

Dr. Jacob Kozacki is a board-certified family medicine physician and native of Florence, SC. He graduated from Clemson University and attended medical school in Spartanburg, SC, at Edward Via College of Osteopathic Medicine. He subsequently returned to his hometown and completed his family medicine residency training at Mcleod Regional Medical Center. Dr. Kozacki is committed to practicing evidence-based medicine and exceptional communication with his patients. This, in turn, contributes to improved patient outcomes and satisfaction. Dr. Kozacki's clinical interests include chronic disease management, acute care medicine, mental health, and providing patients with personalized, up-to-date care plans.
