Jacob Scott Kozacki, D.O.

Dr. Jacob Kozacki is a board-certified family medicine physician and native of Florence, SC. He graduated from Clemson University and attended medical school in Spartanburg, SC, at Edward Via College of Osteopathic Medicine. He subsequently returned to his hometown and completed his family medicine residency training at Mcleod Regional Medical Center. Dr. Kozacki is committed to practicing evidence-based medicine and exceptional communication with his patients. This, in turn, contributes to improved patient outcomes and satisfaction.
